 Parents of children who have reached their fourth birthday by September 30^th may register their child for Pre-Kindergarten for the 2010-2011 school year through Friday, April 30, 2010. Children will not be admitted to Pre-Kindergarten if they reach their fourth birthday after September 30, 2010.  

To register your child for the Meadows of Dan Elementary, Stuart Elementary, or Woolwine Elementary PreK class, please bring the following information to the school:

  

1.                 Certified Copy of Child's Birth Certificate or affidavit

 

2.                 Child's Social Security Number

 3.                 Certificate of Physical Examination performed no earlier than twelve (12) months prior to the date of school entrance and signed by a licensed physician, a licensed practitioner, or a licensed physician’s assistant.

4.                 Record of adequate immunization.  Adequate immunizations include:·         DPT     -2 doses (minimum) ·         Polio     -2 doses (minimum) ·         Measles     -2 doses with one after the first birthday and one prior to entering                      Pre-Kindergarten·         Rubella     -1 dose after the first birthday ·         Mumps     -1 dose after the first birthday ·         Hepatitis B-a minimum of 3 doses for all children born on or after January 1, 1994·         Varicella     -all susceptible children born on and after January 1, 1997, 1 dose not  earlier that 12 mos. of age *

 Those children who have had varicella disease  must have date of disease documented by physician or practitioner.
