Dear Parents/Guardians of Rising 6th Graders: TDAP or Tetanus, Diphtheria, and Pertussis (Whooping Cough) vaccine is required for all students entering into the 6th grade for the 2010-2011 school year. In order for your child to start the first day of school of the upcoming 2010-2011 school year your child must have been vaccinated with the TDAP. A parent must accompany his/her child to the appointment and must bring a copy of all previous shots, as well as a copy of their insurance card. lfyou have any questions or concems please contact your school nurse, local Health Department, or your child's physician's office.
